Add RP configuration setting to require shared associations to be successfully created

Description

The default behavior of the OpenID RP is for dumb mode to be used if a shared association cannot be created with the OP. For security policy reasons, the RP may want to require that a shared association be available in order to be assured of the strength of the secret used in signing. Let's add a configuration option to do that.
